Certificate of appointment to the office of assistant surgeon in the Ohio Volunteer Forces and a letter, directing Silas E. Sheldon to report to Cincinnati, were signed May 13, 1862 by Ohio Governor David Tod. Notification of appointment as asst. surgeon in the 32nd Ohio Volunteer Infantry was sent from Ohio Adjutant General Chas. W. Hill.
